Box Business planにUpgradeしようとして入力フォームに入力したがエラーがでる

Box Business planにUpgradeするにあたり、

Upgrade checkup formに住所やクレジットカード情報を入力したが、

要求の処理中にエラーが発生しましたとでて、

何が割るのかわからないので、非常にこまっている。

早急にアップグレードしてセキュリティを多要素認証の環境に移行したい。
