Upload buttons are gone

I had access to TWO "upload" buttons.  Now I have neither.  Please help me fix this as as soon as possible.

  Hit it New+ button.  It has been relocated there.

  I tried that with no suucess ... it looked like it worked but file did not upload ?

  The upload button is located under the New+ button.  Just click it and it will appear below.
