AWS Lambda addAppUser socket timeout

I have a box enterprise account (TRIAL) with managed users. The flow is AWS -> Cognito User Pool -> Post authentication Lambda which then first this code

This lambda uses the node code from the box samples github repo for create app user lambda.
Why would addAppUser be running longer than 5 seconds, after which Lambda kills it:
Task timed out after 5.01 seconds
The timeout has started recently, no changes to the box app config nor the lambda. Perhaps my trial has run out?
Where can I see logs of what is happening within box for that addAppUser call?

I can confirm that this is an issue when addAppUser is called the 2nd time for a user, for example
1. User A existed -
1.1 When the Cognito Post auth lambda fires addAppUser is called. User exists so I get timeout
2 User B doesn't exist -
2.1 When COgnito Post auth lambda fires addAppUser is called and no error. Box managed user is created
3. User A (above) is deleted) just from Box Managed users, not cognito user pool
3.1 Post auth lambda fires addAppUser, no error. Log out, log back in, Post auth lambda fires addAppUser - socket error.
