Saving Same File for Future Signature

Would seem relatively easy, but new user here test driving box.com's signatures. It seems like whenever I include a field on an uploaded form, e.g., name, address, signature, date, etc., it doesn't save those fields that were inserted. Would seem easy enough to figure out but I'm missing it. If I send the same form or application all the time to different people, is there a way to save that form so the exact fields that were required to be used by the applicant are saved to just point, click and send to new applicants? Shouldn't have to always reinsert the same fields on the same application, and I'm sure I'm just missing how to save it. Thank you...
