My team and I are trying to collaborate on a document, and we all (five people) lost access. Is anyone else experiencing this issue?

We have received similar reports and our engineering team confirmed that there was an issue with where users were unable to login to Box and/or access Box Notes between 7:38 AM to 9:08AM US Pacific time so you may have been affected by this if you tried accessing Box during this time. I'm happy to confirm that this issue has been resolved so you should now be able to access Box and your shared contents again.
