File Request Button Not Showing

Hello - I was using the file request upload widget on my website and it's now disappeared.  I have a paid personal account but cannot access this feature.  What can I do to create an uploaded form on my website like I used to have?  Thank you (:

  Please note that File Request is available to anyone with a Box Business Plan account.
