Two-factor authentication that was not set up has been enabled, and I can no longer log in. 
I don't have a backup code.
Please disable two-factor authentication.
*Since I cannot log in to BOX, I am raising this ticket on your behalf.

•    The last date your account was successfully logged in to
　→don't know
•    The date of account creation
　→don't know
•    Primary email address on file for the account
　<personal information-redacted>
•    Specific names of one or more files associated with the account (owned or collaborated)
　→Add file
