Box Forms is available only for Enterprise Advanced accounts. Customers must agree to the Box Forms Addendum in order to use Box Forms.

Box Forms in Relay lets you choose an existing Form to get started. You can then use this form as a trigger and use the form fields inputs from it to guide your Relay workflow.
Box Apps

With Box Apps, you can use an existing form to collect information within that app. Additionally, If the form used is also a trigger in a Relay workflow, you can trigger that workflow from the app itself. This functionality makes it easier to gather information in a workflow.
Box Doc Gen

In a Relay workflow, Box Doc Gen uses form submissions to collect information, which is then used to generate a document from a template.
